Award for Outstanding Performance in Specific Discipline: Nomination and Evaluation Forms

DepEd Order No. 36 s. 2016 allow schools to bestow awards and recognition on learners who have successfully attained standards set by the school. These awards and recognition support the efforts and accomplishments of the learners and affirm their latent potential, abilities, and dispositions.

One of the awards given at the end of the School Year is the Award for Outstanding Performance in Specific Disciplines under Grade Level Awards.

DO No. 36 s. 2016 sets the qualifications for the awards (pp. 5-7). The downloadable forms below contains detailed criteria for the appraisal of the standards to make the selection of awardees become more objective and impartial.
